  28. Completion of Widows'/Widowers' pension particulars
    1. Original copy of the Marriage Certificate
    2. Documents confirming the manner of termination of previous marriages, if any
    3. Original copy of the spouse's Birth Certificate
    4. Certified copies of spouse's NIC
    5. Original copies of children's Birth Certificates If there are disabled children, a Medical Report and
    6. Certificate issued by a government hospital to confirm that the disability occurred before 26 years of age.
