Complete guide for Sri Lankan citizens to apply for a National Identity Card for the first time.
Every person who is a citizen of Sri Lanka and who has attained or attains the age of 15 years shall apply for a National Identity Card.
Basic Requirements
Who Can Apply?
Every Sri Lankan citizen who has reached the age of 15 years can apply for a National Identity Card.
Main Condition
The application must be submitted through the relevant certifying officer according to the applicant category.
Submitting an Application

It is mandatory for every applicant to forward the application for the National Identity Card through the relevant certifying officer.

Documents to be Submitted Along with the Application
- Duly completed application (Form D.R.P/1,7,8)
- Birth certificate or extract of the birth registry certified by the Additional District Registrar
- ICAO standard photograph obtained within 06 months
- Receipt of payment of application fee

- නිවැරදිව සම්පූර්ණ කරන ලද ඉල්ලුම්පත්රය (පු.ලි.දෙ./වි/1,7,8 ආකෘති පත්රය)
- අතිරේක දිස්ත්රික් රෙජිස්ට්රාර් විසින් සහතික කරන ලද උප්පැන්න සහතිකය හෝ උප්පැන්න ලේඛණයෙන් උපුටාගත් සටහන
- මාස 06ක කාලයක් ඇතුළත ලබාගත් ICAO ප්රමිතියෙන් යුතු ඡායාරූපය
- ඉල්ලුම්පත් ගාස්තුව ගෙවූ බවට කුවිතාන්සිය
விண்ணப்பத்துடன் சமர்ப்பிக்க வேண்டிய ஆவணங்கள்
- சரியாக பூர்த்தி செய்யப்பட்ட விண்ணப்பப் படிவம் (ஆ.ப.தி./வி/1,7,8 விண்ணப்பப் படிவம்)
- மேலதிக மாவட்டப் பதிவாளரால் சான்றளிக்கப்பட்ட பிறப்புச் சான்றிதழ் அல்லது பிறப்புப் பதிவேட்டில் இருந்து பிரித்தெடுக்கப்பட்ட குறிப்பு
- 06 மாத காலத்திற்குள் பெற்றுக்கொள்ளப்பட்ட ICAO தரமுடைய புகைப்படம்
- விண்ணப்பக் கட்டணம் செலுத்தியதற்கான பற்றுச்சீட்டு
