Applications are invited for recruitment on a contract basis to fill vacant positions in the North Central Provincial Department of Health Services.
Applications are invited from:
- Public Service officers who retired after reaching the age of 60 years; and
- Officers of the Tri-Forces who retired after completing the prescribed period of service.
Applicants must send their curriculum vitae together with the following documents:
- Letter expressing consent
- Medical certificate obtained within one month
- Certified copy of the National Identity Card
- Copy of the birth certificate
- A certificate from the institution which last maintained the applicant’s personal file, confirming that no disciplinary action has been taken
- Copy of the retirement letter
- Copy of the pension grant sheet
The curriculum vitae and supporting documents must be sent by registered post to the following address before the closing date:

- කැමැත්ත ප්රකාශ කිරීමේ ලිපිය
- මාසයක් ඇතුළත ලබාගත් වෛද්ය සහතිකය
- ජාතික හැඳුනුම්පතේ සහතික කළ පිටපත
- උප්පැන්න සහතිකයේ පිටපත
- විනය දඬුවම් ලබා නොමැති බවට, අවසන් වරට පෞද්ගලික ලිපිගොනුව නඩත්තු කළ ආයතනයෙන් ලබාගත් සහතිකය
- විශ්රාම ගැනීමේ ලිපියේ පිටපත
- විශ්රාම ප්රදාන පත්රයේ පිටපත
ජීව දත්ත පත්රිකාව සහ අදාළ ලේඛන නියමිත දිනට පෙර ලියාපදිංචි තැපෑලෙන් පහත ලිපිනයට යොමු කළ යුතුය:

வடமத்திய மாகாண சுகாதார சேவைத் திணைக்களத்தில் வெற்றிடமாகக் காணப்படும் பதவிகளுக்கு ஒப்பந்த அடிப்படையில் ஆட்சேர்ப்புச் செய்வதற்காக விண்ணப்பங்கள் கோரப்படுகின்றன.
- 60 வயதைப் பூர்த்தி செய்து ஓய்வுபெற்ற அரச சேவை உத்தியோகத்தர்கள்; மற்றும்
- குறித்த சேவைக் காலத்தைப் பூர்த்தி செய்த பின்னர் ஓய்வுபெற்ற முப்படை அதிகாரிகள்.
விண்ணப்பதாரர்கள் தமது சுயவிபரக் கோவையுடன் பின்வரும் ஆவணங்களை இணைத்து அனுப்ப வேண்டும்:
- விருப்பத்தை வெளிப்படுத்தும் கடிதம்
- ஒரு மாதத்திற்குள் பெறப்பட்ட மருத்துவச் சான்றிதழ்
- தேசிய அடையாள அட்டையின் உறுதிப்படுத்தப்பட்ட பிரதி
- பிறப்புச் சான்றிதழின் பிரதி
- ஒழுக்காற்று நடவடிக்கைகள் எடுக்கப்படவில்லை என்பதை உறுதிப்படுத்தும் வகையில், இறுதியாக தனிப்பட்ட கோவையைப் பராமரித்த நிறுவனத்திலிருந்து பெறப்பட்ட சான்றிதழ்
