The Department of Examinations has requested all school principals to submit information regarding the examination result sheets and certificates received by their schools from 2016 to 2025.
The requested information applies to the following examinations:
Information to Be Submitted
Principals must check the school records and enter the following details separately for the period from 2016 to 2025:
- Number of result sheets and certificates received by the school.
- Number of result sheets and certificates already issued to students.
- Number of result sheets and certificates still remaining at the school.
The above information must be entered accurately in the official online form and submitted to the Department of Examinations within one month.
Result Sheets Must Be Issued Without Delay
The Department of Examinations has instructed principals to issue the available result sheets and certificates to the relevant students as soon as possible.
- Result sheets should not be kept at the school unnecessarily.
- The relevant students should be informed and the documents should be issued without delay.
- G.C.E. O/L and G.C.E. A/L individual result sheets must be properly certified with the principal’s signature and the school’s official stamp before being issued.
- Grade 5 Scholarship Examination certificates should also be issued promptly to the relevant students.
- Accurate records must be maintained for every result sheet or certificate issued to a student.
Principals are responsible for ensuring that the information submitted through the online form is accurate. They must also prevent the alteration, misuse or fraudulent use of result sheets and certificates.

2016 සිට 2025 දක්වා පාසල වෙත ලැබුණු පහත සඳහන් විභාගවල ප්රතිඵල සටහන් සහ සහතික පත්ර පිළිබඳ තොරතුරු විභාග දෙපාර්තමේන්තුව වෙත ලබාදෙන ලෙස සියලුම විදුහල්පතිවරුන්ගෙන් ඉල්ලා ඇත.
- 5 ශ්රේණිය ශිෂ්යත්ව විභාගය
- අ.පො.ස. සාමාන්ය පෙළ විභාගය
- අ.පො.ස. උසස් පෙළ විභාගය
ඇතුළත් කළ යුතු තොරතුරු:
- පාසලට ලැබුණු ප්රතිඵල සටහන් සහ සහතික පත්ර සංඛ්යාව
- සිසුන් වෙත දැනටමත් ලබාදී ඇති සංඛ්යාව
- පාසලේ තවමත් ඉතිරිව ඇති සංඛ්යාව
අදාළ තොරතුරු නිවැරදිව මාර්ගගත පෝරමයට ඇතුළත් කර මාසයක් ඇතුළත විභාග දෙපාර්තමේන්තුව වෙත ඉදිරිපත් කළ යුතුය.
පාසලේ ඉතිරිව ඇති ප්රතිඵල සටහන් සහ සහතික පත්ර අදාළ සිසුන් වෙත ප්රමාදයකින් තොරව හැකි ඉක්මනින් ලබාදෙන ලෙසද විභාග දෙපාර්තමේන්තුව විදුහල්පතිවරුන්ට දැනුම් දී ඇත.
අ.පො.ස. ප්රතිඵල සටහන් විදුහල්පතිවරයාගේ අත්සන සහ පාසලේ නිල මුද්රාව යොදා නිසි ලෙස සහතික කිරීමෙන් පසු සිසුන් වෙත ලබාදිය යුතුය.

சமர்ப்பிக்க வேண்டிய விபரங்கள்:
- பாடசாலைக்குக் கிடைக்கப்பெற்ற பெறுபேற்று அட்டவணைகள் மற்றும் சான்றிதழ்களின் எண்ணிக்கை
- மாணவர்களுக்கு ஏற்கனவே வழங்கப்பட்டவற்றின் எண்ணிக்கை
- பாடசாலையில் இன்னும் மீதமாக உள்ளவற்றின் எண்ணிக்கை
தேவையான விபரங்களைச் சரியாக இணையப் படிவத்தில் பதிவுசெய்து ஒரு மாதத்திற்குள் பரீட்சைகள் திணைக்களத்திற்குச் சமர்ப்பிக்க வேண்டும்.
பாடசாலையில் மீதமாக உள்ள பெறுபேற்று அட்டவணைகள் மற்றும் சான்றிதழ்களை உரிய மாணவர்களுக்கு தாமதமின்றி விரைவாக வழங்குமாறும் அதிபர்களுக்கு அறிவுறுத்தப்பட்டுள்ளது.
க.பொ.த. பெறுபேற்று அட்டவணைகள் அதிபரின் கையொப்பம் மற்றும் பாடசாலையின் உத்தியோகபூர்வ முத்திரையுடன் முறையாக உறுதிப்படுத்தப்பட்ட பின்னரே மாணவர்களுக்கு வழங்கப்பட வேண்டும்.
